An unannounced renewal inspection was conducted today from 10:45am-12:30pm. There were 5 children (16 points) directly supervised by 2 staff. The home, 2 staff records, 3 children?s records, 5 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. Diapering procedures were also observed. There was a supply of books, toys, and materials for the children. Standard #: 22VAC40-111-750-A
Description: Based on observation, the provider did not ensure diaper ointment was administered with no written authorization. Evidence: Child B had a diaper cream which was utilized during a diaper changing but did not have written authorization from parents.

Plan of Correction: We will get form and get it signed and put in the file.

Standard #: 22VAC40-111-830-A
Description: Based on record review, the provider did not conduct emergency evacuation procedures monthly. Evidence: The last documented drill was December 2017. Upon interview, the provider stated January and February drills were not conducted.

Plan of Correction: We will do the drills.

Standard #: 22VAC40-111-70-A
Description: Based on three records reviewed, the provider did not ensure parents received all required information prior to the one child's first day of attendance. Evidence: Child B does not have documentation of the provider's medication policy.

Plan of Correction: We will get forms and have them sign.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not maintain all equipment in a safe condition. Evidence: A light brown crib closest to the play kitchen has a broken wheel and is unsteady posing a safety hazard to the child.

Plan of Correction: We will have it fixed it today.

Standard #: 22VAC40-111-680-A
Description: Based on observation, the caregiver did not wash their hands before serving a child snack. Evidence: Child A was observed to be provided snack and the provider did not wash hands before preparing and serving snack.

Plan of Correction: We will wash hands before and after serving snack.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not ensure a child's hands were washed with liquid soap and running water. Evidence: Child A was observed being served snack and eating snack without washing hands prior to eating.

Plan of Correction: Children will wash hands before and snack.
