Add: A Subsidy Health and Safety inspection was conducted on 3/20/2018 to review supplemental health and safety requirements. There were six children in care with the licensee. The point total for the six children was 15. Five children's records were reviewed, the provider's record was reviewed, assistant caregiver's and the household member's record was reviewed. No medications at the family day home. The inspector arrived at the provider's home at 9:20 am and departed at 1:00 pm. The children were observed during free time, during a music and movement activity, story time, during lunch, and during nap. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.Virginia Quality is a voluntary quality rating and improvement system for early care and education facilities serving children ages birth through pre-K.
