Standard #: 22VAC40-111-70-A
Description: Based on a sample review of children's records, the Provider failed to provide parents in writing the following information: amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider. Evidence: Four of five reviewed records were not complete with both assistants' names or the amount of time scheduled weekly to care for the child instead of the provider.

Plan of Correction: The provider will complete the name and schedule of each assistant and or substitute provider in each child's record. Each parent will be given a completed copy. Documentation will be maintained in each child's record, dated and initialed by the child's parent.

Description: Based on direct observations, the caregiver did not ensure proper hand washing procedures for children, using liquid soap and warm running water, were followed after toileting or diapering procedures. Evidence: The LI observed a caregiver use a moist wipe to clean a child's hands after a diapering procedure.

Plan of Correction: After all diapering procedures, caregivers will ensure children's hands are washed at a sink using liquid soap and running water. Hand washing procedures will be reviewed with all caregivers.

Description: Based on direct observations, a caregiver failed to clean and sanitize the diapering surface after each use. Evidence: 1. The LI observed caregiver #3 during a diapering procedure. 2. Upon completion of the diapering procedure and placing the child down in the floor to play, the caregiver left the area to wash her hands. The caregiver returned, picked up an infant child and sat in the floor with the child. 3. The caregiver did not clean and sanitize the diapering surface. Upon interview, the caregiver reported that she would have cleaned/sanitized the changing surface prior to changing another child. The LI reviewed the diapering procedures with the caregiver. The caregiver cleaned and sanitized the diapering surface upon notice.

Plan of Correction: All caregivers will clean and sanitize the diapering surface after each use and prior to washing her hands.
