Comments:
An unannounced monitoring inspection was conducted on April 5, 2018 from approximately 10:20 am to 2:10 pm. Provider and assistant were present with eight children, two of which are not enrolled. Children were present playing freely, eating lunch and napping. Five children's records were reviewed. The records for provider, assistant and household member were also reviewed.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on review, provider did not ensure 1 of 5 children's records is reviewed and updated annually by parents. Evidence: On April 5, 2018, the record for child #5 was last reviewed and updated by parents on April 8, 2016.

Plan of Correction: Parent has the package to complete.

Standard #: 22VAC40-111-60-B
Description: Based on review, provider did not ensure 3 of 5 children's records has documentation of required information. Evidence: On April 5, 2018, the following was found during record review: 1. The record for child #2 (start date March 9, 2018) does not have a signed medical authorization from parents. 2. The record for child #3 (start date January 30, 2017) does not have a signed medication authorization from parents. 3. The record for child #4 does not have documentation of start date, parent's home information, parent's employment information, two emergency contacts, physician's address and persons authorized to pick up the child.

Plan of Correction: One parent already has a package to update the information. Parents will be asked to signed medical authorizations.

Standard #: 22VAC40-111-70-A
Description: Based on review, provider did not ensure 1 of 5 children's records has documentation of notifying parents whether there is liability insurance coverage or not. Evidence: On April 5, 2018, the record for child #5 (start date April 11, 2016) does not have documentation of parents notification of liability insurance coverage and provisions of emergency preparedness plan.

Plan of Correction: Parents have been given the forms to sign.

Standard #: 22VAC40-111-80-A
Description: Based on review, provider did not ensure 1 of 5 children's records has documentation of viewing proof of child's identity and age. Evidence: On April 5, 2018, the record for child #2 (start date January 30, 2017) does not have proof of child's identity and age documentation.

Plan of Correction: Law enforcement will be notified and it will be documented.
