An unannounced monitoring inspection was conducted today, 4/13/2018. Children's records and HHM records were reviewed during the inspection. There are no day care children in care during today's inspection. A violation notice was completed regarding the violations found during the inspection. Standard #: 22VAC40-111-80-A Description: Based on review of records the provider failed to obtain proof of identity within seven days of child's first day of attendance. Evidence: 1. Child # 4 began care on 2/22/18 and there is no proof of identity on file. Plan of Correction: The provider will talk with the parent and then call the local police department and report this and then document when the call is made.
