Two child record and four staff records were reviewed.

Standard #: 22.1-289.035-A Description: Based on a review of four staff records, the facility failed to ensure that all staff complete a repeat fingerprint background check at 5 year intervals.
Evidence:
1. There are no repeat fingerprint background checks on file for staff # 1-4.Plan of Correction: The program director reported that all fingerprint background checks will be completed as soon as appointments can be made.
