An unannounced follow-up monitoring inspection from the initial inspection was completed today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, and supplies were inspected today between 2:30-4pm. There were 5 children (plus the Provider's 2 children), totaling 17 points, under the supervision of 3 caregivers. During the inspection, children were observed waking up from naptime, having diapers changed, and enjoying free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served ground beef, mixed vegetables (tomatoes, potatoes, green beans) with rice and milk. The Provider is not certified in MAT (medication administration training). Standard #: 22VAC40-111-550 Description: Based on observations, the Provider did not follow all the requirements for using a play pen. Evidence 1. The play pens are not be used for the designated sleeping area for children. Upon entering the FDH, the LI observed a child waking up from naptime in a play pen. Plan of Correction: The Provider will not use this for the child's sleeping area.
Standard #: 22VAC40-111-680-A Description: Based on observations, the Provider failed to wash her hands after diapering a child. Evidence 1. Caregiver #1 was observed diapering 2 children without washing her hands in between or afterwards. Plan of Correction: The Provider will wash her hands after diaperiing.
Standard #: 22VAC40-111-680-B Description: Based on observations, the Provider failed to ensure that children's hands are washed after diapering. Evidence 1. Caregiver #1 was observed diapering 2 children. Neither child had their hands washed after their diapers were changed. Plan of Correction: The Provider will wash their hands after diapering.
Standard #: 22VAC40-111-690-D Description: Based on observations, the diapering surface did not meet the requirements. Evidence 1. The diapering surface was not non-absorbent and washable. The surface was of a cloth material. 2. The diapering surface was not cleaned and sanitized after each use. Two children's diapers were changed during the inspection. Due to being an absorbent surface, it was unable to be cleaned (with soap and water) and sanitized (with a bleach solution) after each use. Plan of Correction: The Provider has a non-absorbent diaper mat which she will use. She will also clean and sanitize it after each use.
