Reminded provider that all background checks are now effective for 5 years rather than 3 years.
An Announced Initial inspection was conducted this date. A total of 3 children (10pts) and Provider and 1 assistant was present. When the inspector arrived, the children were observed playing freely on the floor. The home has ample amounts of toys, equipment and supplies that are age appropriate for the children currently in care. The toys are arranged within reach of the children which promotes independence in toy selection. Hand washing procedures were observed prior to lunch. Lunch today was spaghetti. Both provider and assistant was observed sitting with the children while they ate. Walk thru of the entire home including the back deck was conducted. All enrolled children's records, Provider, assistant and household members records were reviewed. No medications and no topical ointments on site at this time. MAT, CPR and First Aid certificates were verified. This initial inspection began at 9:30am and ended at 12:15pm. Thank you for your cooperation today. Please email me at: email@example.com
Standard #: 22VAC40-111-100-A Description: Based on all children's records, one health exam was not obtained within 30 days of enrollment. Evidence: Child #3 began care on 1/8/18. No documentation of a health exam was found in the child's record. Plan of Correction: I will request a health exam from this child's parents.
Standard #: 22VAC40-111-430-B Description: Based on observation of the physical plant, one gate located on the main level was not mounted as required. Evidence: The gate located at the bottom of the stairs on the main level of the home was pressure mounted. Plan of Correction: Provider will hard mount this gate.
