Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and household members must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file.

Comments:
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located near Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (27 points) with 2 caregivers and 1 volunteer. A total of 9 children?s files, 2 staff files were reviewed. The children were engaged in the following: movie time, learning, music, toy play, games, infant sleeping, diaper changing, hand washing, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 9 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (04/17/2018), documentation of a physical examination was not available for review for Child #4. 2. The start date listed for Child #4 was 10/31/2017.

Plan of Correction: A documented physical for Child #4 shall be obtained and placed on file.

Standard #: 22VAC40-111-60-B
Description: Based on review, 2 of 9 children's files reviewed indicated that not all of the files contained all of the required information/documentation within each child's record. Evidence - 1. On the date of inspection (04/17/2018), the policy number was not available for Child #5. 2. On the date of inspection, there were 1 of 2 complete emergency contact information documented for Child #4. The address was not available for one contact. 3. On the date of inspection, the proof of identification was not documented for Child #4. The start date for Child #4 was listed as 10/31/2017.

Plan of Correction: All missing information/documentation shall be obtained and placed on file.

Standard #: 22VAC40-111-90-A
Description: Based on review, 1 of 9 children's files reviewed indicated that the Provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before a child may attend the Family Day Home (FDH). Evidence - 1. On the date of inspection (04/17/2018), documentation of an immunization record for Child #4 was not available for review. 2. The start date for Child #4 was listed as - 10/31/2017.

Plan of Correction: The parent of Child #4 shall be contacted and a current immunization record shall be obtained.

Standard #: 22VAC40-111-90-C
Description: Based on review, 2 of 9 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (04/17/2018), the following children did not have current immunization documentation on file at the FDH: Child #7 (most recent immunization was dated - 06/14/2017), and Child #9 (most recent immunization was dated - 05/05/2017. 2. On the date of inspection, both Child #7 and Child #9 were under two years of age.

Plan of Correction: Current immunization pages shall be obtained for both children and placed on file.

Standard #: 22VAC40-111-140-B-3
Description: Based on review, the Provider is not currently certified in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care. Evidence - 1. On the date of inspection (04/17/2018), the Provider does not hold a current certification in CPR. 2. The expiration date of the most current certification in CPR expired - 08/28/2017.

Plan of Correction: CPR certification shall be obtain.

Standard #: 22VAC40-111-140-B-4
Description: Based on review, the Provider is not currently certified in first aid. Evidence - 1. On the date of inspection (04/17/2018), the Provider does not hold a current certification in first aid. 2. The expiration date of the most current certification in first aid expired - 08/28/2017.

Plan of Correction: First aid certification shall be obtained.

Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 2 staff files viewed indicated that the Provider did not obtain for each caregiver a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most recent documentation on file. Evidence - 1. On the date of inspection (04/17/2018), the most recent TB documentation on file for Staff #2 was dated - 10/13/2015. 2. This same standard was cited previously during an inspection conducted on 10/28/2017.

Plan of Correction: Updated documentation shall be obtained and placed on file for Staff #2.
