Comments:
An unannounced renewal inspection was conducted on Tuesday, April 24, 2018 from approximately 11:15AM to 1:20PM to determine the provider's compliance with licensing standards. In attendance were four non-resident children. The children were being supervised by one caregiver during the inspection. There was a total of nine points based on the children?s ages. The children were observed playing with toys, working puzzles, washing hands and eating lunch. A walk through of the home was conducted with the provider. The child care areas were filled with supplies and toys for the children. During the inspection, the licensing inspector reviewed the following: emergency supplies including first aid kit, battery operated weather radio, emergency plan, emergency drill logs, required postings, menu, caregivers? training hours and daily attendance. The provider stated medications are not being administered at this time. Transportation is not provided. During the inspection five children's and one caregiver?s record and two household members? records were reviewed. The risk profile tool will be used for the licensing recommendation by the licensing department. Standard #: 22VAC40-111-830-C
Description: Based on documentation review and interview, the provider did not ensure all emergency evacuations drills were documented. Evidence: During the inspection, the licensing inspector observed the drill logs did not contain documentation of an emergency evacuation drill for January 2018 and February 2018. The provider stated an emergency evacuation drill was completed in those months but the provider had forgotten to document them.

Plan of Correction: I will make sure I document the drills as they occur.

Standard #: 22VAC40-111-100-A
Description: Based on review of five children's records, the provider did not obtain documentation of a physical exam within the required time frame. Evidence: During the inspection, the licensing inspector observed the records for Child #1, Child #2 and Child #4 (start date 03/13/2018) did not contain documentation of a physical exam by or under the direction of a physician. Documentation of a physical exam is required for each child prior to or within 30 days after the first day of attendance.

Plan of Correction: I will ask the parent for the physical exams.

Standard #: 22VAC40-111-60-A
Description: Based on review of five children's records, five records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the records for Child #1, Child #2, Child #3, Child #4 and Child #5 (start date 03/13/2018) did not contain the address for one of the two emergency contacts listed in the record. The records for Child #1, Child #2, Child #3, Child #4 and Child #5 did not contain the policy number for the child's medical insurance.

Plan of Correction: I will ask the parent for the information today at pick up.

Standard #: 22VAC40-111-90-A
Description: Based on review of five children's records, the provider did not obtain documentation of immunizations prior to children attending the family day home. Evidence: During the inspection, the licensing inspector observed the records for Child #1, Child #2, Child #3, Child #4 and Child #5 (start date 03/13/2018) did not contain documentation of immunizations prior to the children attending the family day home. The records for Child #1 and Child #4 contained documentation of immunizations dated 03/14/2018. The record for Child #3 contained documentation of immunizations dated 03/28/2018. The records for Child #2 and Child #5 did not contain documentation of immunizations.

Plan of Correction: I will ask the parent for the immunizations record.
