An unannounced monitoring inspection was conducted today. There are 8 children present at LI arrival (10:05 AM), over capacity. 8 children's records were reviewed. See violation notice form missing information. Children were observed eating snack, washing hands, playing with toys, during diaper changes and dancing. Standard #: 22VAC40-111-100-A Description: Based upon a review of records, one child's record does not contain documentation of a completed physical within 30 days of enrollment. Evidence: Child 2 does not have a physical on file. (Date of enrollment: 3/2/18). Plan of Correction: I will ask the parent to provide the physical.
Standard #: 22VAC40-111-40-A Description: Based upon direct observation, the provider exceeded the licensed capacity of 7 children. Evidence: There are 8 children present at LI arrival at 10:05 AM. Plan of Correction: I will not allow not allow 2 children to come.
Standard #: 22VAC40-111-60-B Description: Based upon a review of records, children's record do not contain documentation of all required information. Evidence: *2 of 8 records reviewed did not contain insurance information. (Children # 6, 3) * 2 of 8 records reviewed did not contain complete proof of birth documentation. No documentation, or the signature and date of the person viewing the documentation. (Children #5, 4, 3) *1 of 8 records reviewed did not contain the child's doctor's information. (Child 3) Plan of Correction: I will ask the parents to provide the documentation.
Standard #: 22VAC40-111-70-A Description: Based upon a review of records, one parent has not acknowledged receipt of the Provision of the Emergency Preparedness Plan. Evidence: There is no signed Provisions of the Emergency Preparedness plan on file. (Child 2) Plan of Correction: I will ask the parent to sign the form.
Standard #: 22VAC40-111-90-A Description: Based upon a review of records, one child's record does not include an immunization record prior to the first date of attendance. Evidence: Child 1 (Date of enrollment: 4/2/18) does not have an immunization record on file. Plan of Correction: I will ask the parents to provide the documentation.
Standard #: 22VAC40-111-570-A Description: Based upon direct observation, the provider failed to follow the allowable point system of 16 points per caregiver. Evidence: At LI arrival there was 20 points with one caregiver. Children's ages: 3yr (2 points), 4- 2yr olds (8 points), 21 month (3 points), 16 month (3 points), 9 months (4 points). Plan of Correction: I will not allow two children to come anymore.
Standard #: 22VAC40-111-690-A Description: Based upon direct observation, a child was left unattended on the changing table.. Evidence: One child was left unattended on the changing table while twice during a diaper change. Plan of Correction: I will not leave the children alone on the changing table.
