Technical Assistance:
Assistance was given today regarding training available through the DSS website, licensing renewal, and the new Central Registry System.

Comments:
Today an unannounced renewal inspection was conducted with provider. This visit took place in the morning between 8:45am-12:00pm. There were 6 children present with the provider and 2 assistant's for a total of 16 points. The children were observed dancing, floor time play, singing with caregiver and eating. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-120-B
Description: Based on review of three staff records, provider did not have documentation that staff received orientation training that includes job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities. Evidence: Caregiver #1 (start date: April 30, 2018) did not have a complete record. Missing were the job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities.

Plan of Correction: She just started yesterday, I will go over it as so as possible.

Standard #: 22VAC40-111-320-A
Description: Based on observation small electrical appliances were plugged in when not in use. Evidence: In the kitchen a toaster and toaster oven were plugged in and were not currently in use.

Plan of Correction: I used the toaster earlier, and will use the toaster oven for lunch, I'll make sure they are unplugged when they are not in use.

Standard #: 22VAC40-111-520-C
Description: Based on observation there is a crib that is not in compliance with the mandatory CPSC safety standards. Evidence: One crib has a manufactured date of 2/19/04. ?Due to the licensee?s non-compliance with the Mandated Federal Crib Standards, the licensee must come into compliance and submit appropriate documentation to the licensing inspector within 30 days of receipt of the violation notice. The licensing inspector and the licensee discussed that any non compliant crib(s) in the home can only be used for the licensee?s own children or children that reside in the licensed family day home. The licensee has identified to the licensing inspector all cribs that will be used for children enrolled in the family day home.?

Plan of Correction: I will not use the crib and either replace the crib or transition a child to a mat when they turn one next week.

Standard #: 22VAC40-111-680-A
Description: Based on observations the caregiver failed to wash her hands with liquid soap and warm running water after diapering a child. Evidence: Caregiver was observed changing two different children and failed to wash her hands between diapering each child.

Plan of Correction: I will speak with her today and make sure she washes her hands with soap and water.

Standard #: 22VAC40-111-680-B
Description: Based on observation caregiver did not wash the children's hands with liquid soap and warm running water. Evidence: In two different occurrences, children were observed not having their hands washed after a diaper change.

Plan of Correction: I will speak with her today and make sure she knows to wash the children's hands.

Standard #: 22VAC40-111-690-D
Description: ased on observation the diapering surface was not cleaned and sanitized after each use. Evidence: Caregiver did not clean and or sanitize the diapering surface after each diaper change.

Plan of Correction: I will speak with her today and make sure she cleans and sanitizes after each diaper change, she started yesterday.

Standard #: 63.2-1720.1-A
Description: Based on review of three staff records, provider did not maintain up to date records all of the required background checks. Evidence: Caregiver #1 (date of hire: 4/30/18) did not have the required fingerprint check completed.

Plan of Correction: She is new and just started yesterday, I will have her to do right away.
