An unannounced Renewal Inspection was conducted on October 25, 2023 between the hours of 10:15am and 11:55am. There were classrooms observed with a total of 45 children present ranging in ages from 2 to 5 years with 8 staff supervising. Ratios were in compliance.
The children were observed playing on the playground, singing songs, manipulatives, toileting routines, hand washing procedures, and other organized activities. A complete inspection of the physical plant, child and staff records, fire drill log, and emergency procedures were observed during this inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22.1-289.035-B-4 Description: Based on record review and interview, the center failed to obtain an out-of-state Child Abuse and Neglect Search Request by the end of 30th day of employment.
Evidence:
1. Staff #12 did not have documentation of an out-of-state child abuse and neglect search result in the file.
2. Staff #12 moved to Virginia in 2019.
3. The director acknowledged the out-of-state search had not been completed.Plan of Correction: I will submit the central registry search today. I will let you know when I receive the results.
Standard #: 22.1-289.035-B-4 Description: Based on record review and interview, the provider did not obtain an out-of-state criminal history record check prior to the 1st day of employment.
Evidence:
1. Staff #12 (DOH 8/30/23) does not have documentation of an out-of-state criminal history check on file.
2. Staff #12 moved to Virginia in 2019.
3. The director acknowledged the out-of-state criminal history search had not been completed.Plan of Correction: I will submit the criminal history search right now.
Violation was corrected during the inspection and search results were received within the hour of inspection.
