Technical Assistance:
The inspector and provider discussed better record keeping procedures.

An unannounced monitoring inspection was conducted today from 11:15am-1:30pm. There were 6 children (17 points) directly supervised by 2 staff. The home, 1 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating lunch. Lunch served today was cucumbers, carrots, a sandwich and milk. Diapering procedures were also observed. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-B
Description: Based on record review, the provider did not practice shelter in place procedures a minimum of twice per year. Evidence: In 2017, there were no shelter in place drills documented. Upon interview, the provider stated drills were conducted but there was no documentation to support.

Plan of Correction: I will document the drills.

Standard #: 22VAC40-111-60-B
Description: Based on four records reviewed, the provider did not maintain complete files for each enrolled child. Evidence: 1) Child A does not have an work address and phone number for one parent. 2) Child B and C do not have street addresses for both emergency contacts listed and a medical insurance policy number.

Plan of Correction: Today I will talk to parents.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain from each adult household member a current report of tuberculosis screening form every two years. Evidence: Household member A has a tuberculosis screening which was conducted in April 2016.

Plan of Correction: The household member will make an appointment and update the screening.

Standard #: 22VAC40-111-430-A
Description: Based on observation, the provider is not sufficiently preventing access to stairs. Evidence: The provider has a gate bolted to the wall that is not in working order and is propped open. Additionally, the provider is using a pressure mounted gate to block access to the stairs which is place in front of the stairs and not locked in place using pressure. Access to the stairs is not prevented by the gates used by the provider.

Plan of Correction: We will fix the gate.

Description: Based on record review, the most recent inspection findings were not posted. Evidence: The most recent inspection conducted was on 10/17/2017. The violation notice posted was from an inspection conducted on 5/18/2017.
