Technical Assistance:
Changes in the background check regulations were reviewed with the provider. A sample sworn disclosure or affirmation form was given to the provider.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 1:45 PM and departed at 3:00 PM. Three children, totaling seven points, were in care with one caregiver. Two caregiver/household records and four children's records were reviewed. It was reported that no children are administered medication. No children's injuries were reported. An exit meeting was conducted with the provider prior to closure of the inspection.

Standard #: 22VAC40-111-180-A
Description: Based upon review of two caregiver/household records, the provider has not ensured that each caregiver and adult household member obtains a tuberculosis screening every two years. Evidence: 1. The most recent tuberculosis screening in the record for caregiver/household member 1 is dated 03/16/2016. 2. The most recent tuberculosis screening in the record for caregiver/household member 2 is dated 03/16/2016. 3. The provider acknowledged that repeat tuberculosis screenings are needed for caregiver/household members 1 and 2.

Plan of Correction: The provider responded with the following: Tuberculosis screenings will be obtained every two years for caregiver/household members.
