Comments:
A Subsidy Health and Safety inspection was conducted on June 4, 2018 to review supplemental health and safety requirements. The inspection was initiated at 10:30 am and concluded at 1:00 pm. Upon the inspector's arrival, nine children were in care with two caregivers for a value of 24 points. Shortly after an additional caregiver arrived. Children were observed singing and dancing to a music video. Songs identified body parts and children pointed to them as they sang along. The three infants were sleeping at the time of the inspector's arrival. Later children had time outside to play with small trikes and toys. Upon returning inside children had bathroom breaks, diapering was conducted and hand washing. Lunch time was also observed. Today's lunch consisted of pasta, tomato sauce, mozzarella cheese, applesauce, mixed vegetables and milk. All areas used for child care were inspected along with a walk through of the entire home. The family day home has age appropriate materials and supplies. The provider had the following required information posted: The license, last inspection notice, emergency numbers, allergies, menu, and attendance sign in and out sheets. The first aid kits and non-medical emergency supplies were inspected and found complete. The inspector interacted with caregivers and children in care.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Three caregivers records and nine children's records were reviewed.
The provider has current MAT. Medications, allergy action plans were reviewed.
The most recent evacuation drill was documented on 05/21/2018. The two required shelter-in-place drills for 2017 were completed on 06/29/2017 and 12/11/2018. A lock down drill was completed on 06/29/2017.

Transportation is not provided.
The emergency preparedness plan was reviewed and current.

Violations:
Standard #: VENDHOM-000-012-B
Description: Based on review of caregiver records, the provider did not ensure caregivers completed the Virginia pre-service training within the first 90 days of employment. Evidence: Caregiver #3, hired 02/05/2018, did not have documentation of completing the Virginia pre-service raining exceeding the 90 days from employment.

Plan of Correction: Per the provider: The caregiver has started the training and will complete it by Friday.

Standard #: VENDHOM-000-012-E
Description: Based on review of caregiver training, the provider did not ensure caregivers obtained CPR and First aid training within 30 days of employment. Evidence: Caregiver #3, hired 02/05/2018, did not have documentation of current CPR and first aid training.

Plan of Correction: Per the provider: The caregiver has current CPR and first aid training however I have not gotten a copy for her record. I will have her provide documentation of the current CPR and first aid training.
