Technical Assistance:
This inspection was a follow-up to an inspection originally conducted on 03/26/2018. The Family Day Home currently is on a Provisional License. During this 6 month period the Licensing Inspector shall conduct an Enforcement Watch focusing on those specific areas of the standard in which the FDH has struggled with in the past.

Comments:
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the staff of Adelaide Nyamedi FDH located near the town of Waterford, Virginia. This unannounced Non-mandated Monitoring Inspection (NMI) was conducted as a result of an enforcement watch for the FDH's current Provisional License. Evidence was gathered through the review of documentation and the interview process. There were a total of 7 children (17 points) with 2 caregivers. A total of 7 children's files, 2 staff files, and 2 household files were reviewed. Children were observed engaged in the following: toy play, large motor skills outside of playground, story time, music, lunch, bathroom break, hand washing, diapering, nap time, and other organized activities. Standard #: 22VAC40-111-60-B
Description: Based on review, 3 of 7 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (06/05/2018), the action to take for the allergy listed was not available for: Child #4, and Child #7. 2. The number for the child's insurance policy was not available for: Child #6. 3. This same standard was cited previously during an inspection conducted on 03/26/2018.

Plan of Correction: All missing information shall be obtained from each applicable parent.

Standard #: 22VAC40-111-70-A
Description: Based on review, 1 of 7 children's files reviewed indicated that not all of the required documentation was contained within each child's file. Evidence - 1. On the date of inspection (06/05/2018), the Information for Parents form was not signed by the Parent of Child #7. 2. On the date of inspection, the Authorization to Administer Medication Policy form was not available for Child #7. 3. On the date of inspection, the Provisional form for the Emergency Preparedness & Response Plan was not available for Child #7.

Plan of Correction: The Parent of Child #7 shall complete all applicable documentation.

Standard #: 22VAC40-111-140-B-3
Description: Based on review and interview, the Provider does not have a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care. Evidence - 1. On the date of inspection (06/05/2018), it was determined that the Provider does not currently hold a certification in CPR. 2. The most current CPR certification on file expired on 8/15/2017. 3. The Provider stated (during an interview conducted on the date of inspection) that she is scheduled this course for 06/23/2018 and that this class was the earliest available. 4. This same standard was cited previously during an inspection conducted on 03/26/2018.

Plan of Correction: A class is scheduled for next month.

Standard #: 22VAC40-111-140-B-4
Description: Based on review and interview, the Provider does not have a current certification in First Aid. Evidence - 1. On the date of inspection (06/05/2018), it was determined that the Provider does not currently hold a certification in First Aid. 2. The most current First Aid certification on file expired on 8/15/2017. 3. The Provider stated (during an interview conducted on the date of inspection) that she is scheduled this course for 06/23/2018 and that this class was the earliest available. 4. This same standard was cited previously during an inspection conducted on 03/26/2018.

Plan of Correction: This class is scheduled for next month.

Standard #: 63.2-1720.1-A
Description: Based on review and interview, 1 of 2 staff files reviewed indicated that an employee did not undergo a background check in accordance with subsection B prior to their employment in the Family Day Home (FDH). Evidence - 1. On the date of inspection (06/05/2018), Staff #2 had a fingerprint check eligibility letter (dated - 05/01/2018) on file at the FDH. However, this check was conducted by the previous employer of Staff #2 and not the current FDH. 2. The Provider stated that she did not know that this letter was non-transferable.

Plan of Correction: A new background check shall be obtained by Staff #2. Staff #2 will not return to the FDH until this information is on file.

Standard #: 63.2-1809.1-B
Description: Based on review, 2 of 7 children's files reviewed indicated that the written documentation verifying the parents review of the liability coverage (if any) held by the Family Day Home (FDH) was not available. Evidence - 1. On the date of inspection (06/05/2018) the documentation of the review of the FDH's liability cover was not available for: Child #5, and Child #7. 2. These documents were not included in each listed child's file.

Plan of Correction: The Parent of each applicable child shall complete this documentation.
