Licensing Inspector Anderson conducted an unannounced Monitoring inspection in response to a self report of an incident that occurred at the center on October 11, 2023. Licensing Inspector initiated the investigation on October 12, 2023 and conducted the onsite inspection on October 18, 2023. Findings were reviewed with the center on October 31, 2023. The Licensing Inspector reviewed 1 child record and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Violations found during the inspection were unrelated to the information submitted by the center.

Standard #: 22.1-289.031-A-4 Description: Based on record review, the Center failed to ensure each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.

The Sworn Disclosure document in the record for Staff #1 (date of hire 8/2023) indicated that the staff person has lived outside of the state of Virginia within the previous 5 years.
