Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and board officers/household members must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. If you did not receive your Facility ID number, your Field Print code numbers or you have not establish a point of contact, then please contact the Office of Background Investigations (OBI) at 1 (804) 726-7884. Discussion that a fingerprint check, a signed sworn disclosure statement, and TB documentation will need to be completed and on file for Household Member #2 one month prior her 18th birthday. In addition, a CPS check will also need to be completed and on file for Household Member #3 one month prior her 14th birthday.
The following inspection took place between the hours of 1:30 pm through 3:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 1 child (2 points) in care with 1 caregiver. A total of 5 children?s files, 1 staff file, and 4 household member?s files were reviewed. The child was engaged in the following: learning about the letter "A," crafts - head bands, learning about shapes - circles, bathroom breaks, hand washing, toy play, nap time, snack, and other organized activities. Also, upon arrival to the FDH, another child was observed being picked up by the child's parent. If you should have any future questions, please call. Thank you.
