During the inspection, the licensing representative discussed with the provider procedures for cleaning and sanitizing the diapering surface, proof of identity and documentation of a physical examination needed for a child recently enrolled, a fire place inspection to be scheduled in the near future and standards regarding the limited use of media to two hours a day.
- Comments:

An unannounced monitoring inspection was conducted today from 10:50am-12:50pm. A total of 9 children were present at the time of arrival with 2 caregivers; however a 10th child arrived during the inspection [totaling 22 points]. Children were observed while potty training, engaged in play with various toys available to them, during lunch and transitions to nap time. A sample of 6 children's records, 5 household/caregiver records, emergency drills, emergency plan and other documents were reviewed. Areas of noncompliance are documented on the violation notice. Standard #: 22VAC40-111-70-A Description: Based on a review of records, the parents of 1 child were not provided the required written information before the child's first day of attendance. Evidence: The record for Child #1 [date of enrollment: 6/4/2018] was missing medication administration policies, declaration of liability insurance, provisions for the emergency response plan, and parent agreement. Plan of Correction: [Requested documents were] received signed with required written information completed from parent of Child #1 upon pick up at the end of child #1's first date of attendance on 6/4/2018.
Standard #: 22VAC40-111-90-A Description: Based on a review of records, 1 of 6 children's records were missing documentation of immunizations prior to the child's first date of attendance. Evidence: The record for Child #1 [date of enrollment: 6/4/2018] did not include documentation of immunizations. Plan of Correction: Documentation of immunizations for Child #1 [date of enrollment: 6/4/2018] were received from parent of Child #1 upon pick-up at the end of child #1's first date of attendance on 6/4/2018.
Standard #: 22VAC40-111-320-B Description: Based on observation, child resistant protective covers were not installed on all unused electrical outlets accessible to children under five years old. Evidence: A protective cover was not installed on an unused outlet in the kitchen above a bench at the table where children [under the age of five years old] were observed eating lunch. Standard #: 22VAC40-111-510-C Description: Based on observation, occupied cribs and rest mats were not spaced at least 12 inches from each other. Evidence: 3 occupied rest mats spaced parallel to one another were not spaced 12 inches apart; An occupied rest mat placed in between two occupied cribs were not spaced at least 12 inches apart. Plan of Correction: Corrected during inspection; A unoccupied rest mat was removed to create more space.
