- Technical Assistance:
The provider and inspector discussed ensuring compliance with the standards, definition of cleaning and sanitizing, directed the provider to resources on the DSS website.
An announced initial inspection was conducted today from 10:15am-12:00pm. There were 11 children (23 points) including the provider's own two children, directly supervised by 2 caregivers. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, playing with playdough, watching television and eating lunch. There was an abundant supply of books, toys, and materials for the children. Standard #: 22VAC40-111-800-A Description: Based on record review, the provider did not have a written emergency preparedness response plan. Evidence: The provider did not have a written emergency preparedness and response plan. Plan of Correction: We will write up the emergency preparedness and response plan.
Standard #: 22VAC40-111-920 Description: Based on record review, the menu posted by the provider does not meet all requirements. Evidence: The menu posted is not dated. Plan of Correction: We will date the menu.
Standard #: 22VAC40-111-30-A Description: Based on observation, the provider did not ensure compliance with the standards issued by the department and with relevant federal, state, or local laws, and other relevant regulations. Evidence: The provider had 9 children in care not including the provider's two children. The provider was caring for more than 4 children prior to the license being obtained per General procedures. Plan of Correction: We want to be licensed. If we do not get licensed, we will reduce numbers.
Standard #: 22VAC40-111-60-B Description: Based on four records reviewed, the provider did not ensure all children's files were complete. Evidence: 1) Child B and C are missing doctor name, phone number, two emergency contacts, authorization to pick up, medical insurance, medical insurance policy number, and authorization for emergency medical care. Plan of Correction: We gave a few parents the paperwork and we will follow up with getting the information.
Standard #: 22VAC40-111-70-A Description: Based on four records reviewed, the provider did not ensure all required written information to parents prior to the child's first day of attendance. Evidence: 1) Child A is missing check in and our procedures, daily schedule, and the discipline policy. 2) Child B and C are missing operating hours, schedule of feed, check in and out procedures, policies for medication, policy for liability insurance, information regarding all requirements for children, provisions for the emergency plan, and termination policies. Plan of Correction: We gave a few parents the paperwork and we will follow up with getting the information.
Standard #: 22VAC40-111-80-A Description: Based on record review, the provider did not obtain documentation of the child's identity. Evidence: Child A, B, C, and D are missing documentation of the proof of identity of the child. Plan of Correction: We viewed originals but did not document. We will document going forward.
Standard #: 22VAC40-111-330-A Description: Based on observation, the provider did not have a landline telephone. Evidence: The provider does not have a landline telephone available. Plan of Correction: Our phone line is a digital phone line. We will follow up on getting a landline telephone.
