54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
The Virginia State Police criminal history record check is no longer required due to national fingerprinting. New staff need to have fingerprinting results prior to hire. The search of the central registry (CPS check) is due to be mailed within 7 days of hire and returned within 30 days. All caregivers and household members over the age of 18 must complete fingerprinting no later than September 30, 2018. Background checks are now valid for five years from the original date the check was completed.

An unannounced monitoring inspection was conducted with two assistants. There were ten children present, ranging in age from 17 months to 10 years of age, totalling 15 points. The older children were watching an age appropriate movie while the younger children had rest time. All the children woke for afternoon snack and a coloring activity. Four household files and two staff files were reviewed - please see Supplemental Summary for missing items. All caregivers are current in first aid and CPR. Five children's files were reviewed and are in compliance. There are no medications for day care children on site - Medication Administration Training is required to administer medication to day care children. Description: Based on review of four household members and two staff files, the provider failed to have a current tuberculosis test for all household members. Evidence: Household member 2 and Household member 4 did not have documentation of a current tuberculosis test or screening.

Plan of Correction: Household members will get documentation of a current tuberculosis test or screening.

Description: Based on review of three household files and two staff files, the provider failed to have repeat sworn disclosure statement for each household member. Household member 2 did not have repeat sworn disclosure statement.

Plan of Correction: Household member 2 will complete a repeat sworn disclosure statement.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on review of four household files and two staff files, the provider failed to have documentation of background checks on file for each person living in the home of the licensed family day home. Evidence: Household member 4 moved into the house 12/31/2017 and does not have documentation of a sworn disclosure statement, criminal history record or fingerprinting, or search of the central registry (CPS check).

Plan of Correction: Provider will obtain background checks for Household member 4.
