Technical Assistance:
Discussed safe sleeping practices for infants. The license cannot be processed for renewal until the cited background check is viewed by the Inspector. Although the provider was not present during this inspection, the Inspector spoke with her on the telephone to make her fully aware of the results of this inspection.

Comments:
An unannounced full renewal inspection was conducted today. The provider was not home during this inspection. The substitute provider and one assistant caregiver were caring for 5 children, totaling 16 points. Background checks, caregiver records, children's records, household member records, children's nutrition, emergency preparedness items, the physical plant, and programming were inspected today. The children were observed playing with toys, interacting with each caregiver, coloring pictures, singing songs, listening to stories, and preparing for lunch. The children were to be served chicken tenders, rice with broccoli, and milk for lunch later today. This inspection was completed between approximately 9:00am and 10:50am. Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver health records, it was determined that the provider has not obtained a repeat TB Health Screening within 2 years from the date of her most recent screening. Evidence: The provider's TB Health Screening expired on 2/8/18.

Plan of Correction: I will get a new screening done tomorrow.

Standard #: 22VAC40-111-340-C
Description: Based on observations, it was determined that the children's bathroom was not equipped as required. Evidence: Upon arrival, there was no toilet paper available for use in the children's bathroom.

Plan of Correction: The toilet paper was immediately replaced during the inspection.

Standard #: 22VAC40-111-530-F
Description: Based on observations, it was determined that soft bedding was placed inside a crib occupied by an infant. Evidence: Upon arrival, the Inspector observed a 5 month old infant sleeping in a crib with a loose blanket gathered at the foot of the crib mattress.

Plan of Correction: The blanket was immediately removed from the crib.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on a review of household member background checks, it was determined that one required background check was not completed. Evidence: One household member recently reached the age that requires a completed Child Protective Services (CPS) registry search. This household member reached this age in February, 2018. The provider has not submitted the CPS background search request.

Plan of Correction: I was not aware that the CPS search is required at this age. I thought it wasn't required until age 18. I will submit the CPS search request form tomorrow.
