An unannounced renewal study was conducted today at the Family Day Home from 10:10am-1:00pm. A total of 11 children [23 [points] were in the care of 3 staff. Children were observed during various learning and organized activities, diapering/toileting, handwashing, lunch and transitions to nap time. Homemade chicken, rice, carrots and apples were served for lunch. A sample of 4 caregiver/household member records, 5 children's records, emergency drills, emergency supplies, and other documents were reviewed. Standard #: 22VAC40-111-170-A
Description: Based on a review of records, documentation of a current Report of Tuberculosis Screening was not obtained for 1 out of 4 caregivers prior to coming in contact with children. Evidence: At the time of inspection, documentation of a current report of tuberculosis screening was not available for Staff #2 [date of employment: 6/15/2018].

Plan of Correction: The caregiver has obtained the screening and will return documentation to me today.

Standard #: 22VAC40-111-200-A
Description: Based on a review of records, 1 out of 4 caregivers did not receive the required training by the end of their first week of assuming job responsibilities. Evidence: The record for Staff #2 [date of employment: 6/15/2018] was missing documentation of a completed staff orientation.

Plan of Correction: Completed during inspection.

Standard #: 22VAC40-111-240-A
Description: Based on observation, ares outside the home were not maintained in safe condition. Evidence: 1. 2 protruding bolts were observed on a wooden playhouse jungle gym in the outdoor play area. 2. Cracked plastic was observed on the windshield of a riding car.

Plan of Correction: I will remove the riding car. I will have the nail/screw tightened.

Standard #: 22VAC40-111-320-H
Description: Based on observation, flammable and combustible materials were observed accessible to children. Evidence: A bottle of lighter fluid was observed on a shelf outdoors near the play area, accessible to children.

Plan of Correction: I will move it to an inaccessible area.

Standard #: 63.2-1720.1-B-3
Description: Based on observation, the record for 1 out of 4 caregivers was missing central registry results from another state in which the caregiver has lived within the past five years. Evidence: A sworn statement for Staff #3 [date of employment: 6/4/2018] disclosed that the caregiver has lived in Texas within the past five years. The record was missing documentation of results for a central registry from that state.

Plan of Correction: I have the documentation filled out and will send it off.
