- Technical Assistance:
Discussed expiring CPR and First Aid certifications. Provider stated that she has signed up for training on7/14/2018. Discussed background check and health screening requirements for household members who are turning 18. Consultation was given on the new background requirements for finger printing, a revised Sworn Statement (SDS) for new staff hired after 7/1/2017 and requesting out-of-state Central Registry checks for staff who have lived outside of Virginia within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out-of-state central registry forms. VA DSS will be paying for initial child care finger print checks until 9/30/2018. Each facility must establish a point of contact with VA DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligibility letters will be sent out by e-mail and staff may not be hired until the completed letter is received. Please read, review, and respond to all correspondences regarding the fingerprint roll out.
An unannounced monitoring inspection was conducted today from 10:00am-11:00am. There were 6 children (16 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today in accordance with the provider's policy. Children were observed playing with blocks, dancing to a laser light show, and working on worksheets. Interactions with caregivers and children were positive and affectionate. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, one of four children's records reviewed was incomplete. Evidence: Child 4's record did not contain a physical address for one of two required emergency contacts. Plan of Correction: I will ask the parents to complete the missing information.
Standard #: 22VAC40-111-180-A Description: Based on record review, not all required adults have submitted the results of a negative Tuberculosis screening at least every two years. Evidence: 1. The most recent Tuberculosis screening for Adult 1 was conducted on 4/19/16. 2. The most recent Tuberculosis screening for Adult 2 was conducted on 4/13/2016. 3. The most recent Tuberculosis screening for Adult 4 was conducted on 6/25/2016. Plan of Correction: I will ensure to have new TB tests conducted for all required persons.
