63.2 Licensure and Registration Procedures

Comments:
Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning there were 7 children with 2 caregivers for a total of 24 points within the supervision guidelines. The children were observed having morning free play story-time, and actively engaged in gross and fine motor activities, lunch and in transition for nap time. Positive interaction were observed. A complete inspection of the physical plant, children and provider records, attendance records, fire drill log, menu and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-920
Description: Based on observation the menu posted did not reflect a date for the current one-week period. Evidence: Upon arrival, the menu posted did not reflect the dates for the current week. Menu posted was for the week of June 18-22nd.

Plan of Correction: Provider immediately corrected the menu.

Standard #: 22VAC40-111-690-D
Description: Based on observation of the diapering area, the diaper changing surface was found with a small rip and no longer nonabsorbent. Evidence: The diaper changing pad had a small rip and hole.

Plan of Correction: Provider stated that it will be replaced today.

Standard #: 63.2-1720.1-B-3
Description: Based on interview with Provider and review of staff/household records, the provider did not obtain a Out-of-state Central Registry check for staff. Evidence: 1. Staff #1's Sworn Disclosure Statement confirmed that they currently live/have lived out-of-state. 2. Record for Staff#1 (hired 3/23/2018) did not have documentation of an out-of-state Central Registry check completed prior to employment.

Plan of Correction: Provider stated that staff has a vacation home out-of the state, and will complete the Central registry form for both states.
