Standard #: 22VAC40-111-800-A Description: Based on record review, the provider did not ensure the written emergency preparedness and response plan was up to date and current. Evidence: The emergency preparedness and response plan was not updated to include a new shelter in place location or a new relocation location for the provider's new home. Plan of Correction: I wanted to confirm the shelter in place location with my inspector and I need to note down the new relocation location which is closer the new home.
