Technical Assistance:
The provider shall send documentation of completed training hours to the Inspector before the current expiration date of the license.

An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 6 children, totaling 14 points. One child in care was a resident child. Adult and caregiver records, background checks, children's records, health records, training records, the physical plant, children's nutrition, and emergency preparedness items were inspected today. The children were observed playing outside, enjoying group games, and playing with bubbles. The children were to be served fish sticks, apple slices, potatoes, whole grain bread, and milk for lunch later today. This inspection was completed between approximately 10:00am and 11:40am. Thank you for meeting with me today. If you have any questions regarding this inspection please email me at

Standard #: 22VAC40-111-100-A
Description: Based on a review of children's health records, it was determined that one record was not complete. Evidence: Child record #1 does not include a physical exam report. This child has a documented start date of 7/17.

Plan of Correction: The parent has been asked many times to bring the physical exam. I will talk with the parent today, and indicate that the physical must be returned no later than Monday morning.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one area of the child care space was not maintained in a safe condition as required. Evidence: In the infant nap room, one of the cribs has been placed against the wall in such a way that it is blocking the door from opening completely. Because the door only opens about half way, it is a safety hazard in the event of the need for a rapid emergency exit.

Plan of Correction: The cribs will be moved so that the door can fully open.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
