Technical Assistance:
Discussed with the provider wading activities, playground equipment, making sure that violations are corrected by the date listed on the violation notice.

An unannounced renewal inspection was conducted today between 10:40am to 2:00pm. There were 5 children (12 points) in care of two caregivers. The children were observed doing water play outside, playing with toys and eating lunch. Children were observed washing hands before and after eating lunch and using the bathroom. Medications and emergency drill records were reviewed. 5 children's records and 4 staff/household records were also reviewed. Standard #: 22VAC40-111-240-A
Description: Based on observation, outside areas of the family day home were not maintained in safe and operable condition. Evidence: 1. A plastic slide with steps was being used for the children to use with a wading pool. The top step for the slide on which the children were climbing had a large crack posing an injury hazard. 2. Splintering wood was observed on the wooden outdoor play equipment.

Plan of Correction: I will fix both things. We will try to fix the step by this weekend.

Standard #: 22VAC40-111-260-D
Description: Based on observation, a portable wading pool was being used by children who are not potty trained. Evidence: 1. Child # 1 was observed doing water play in an inflatable wading pool. 2. Child # 1 is not potty trained.

Plan of Correction: We will not be using the pool with children that are not potty trained.

Standard #: 22VAC40-111-480-D
Description: Based on observation, equipment used by the children was not maintained and used in accordance with the manufactureer's instructions. Evidence: 1. The outside play equipment has a rock climbing panel. The open area at the opening above the rock climbing panel was blocked by a plastic child safety gate. 2. The manufacturer's guidelines for the gate state that it is intended to be used indoors at the bottom of stairs and doorways only. 3. The mount type of this gate is pressure mounted. The gate is attached to the equipment using bands and ties which does not comply with the manufacturer's instructions. 4. There is a rope swing that was used to replace the swing that originally came with the playground equipment. The manufacturer's instructions state that the swing is only to be used for family domestic outdoor use. 5. Both items were not in compliance of their listed manufacturer's instructions.

Plan of Correction: I will remove the gate and the swing.
