Several e-mails have been sent regarding the new requirement for fingerprinting. If you have not received them, please go to our website at dss.virginia.gov and review the "Bulletins" under Current providers. The first step is to contact the Office of Background Investigations at 804-726-7884 to get the Fieldprint Code for your family day home. Once the code is obtained, go online to fieldprintvirginia.com and schedule an appointment to go have the electronic fingerprinting done. The locations are listed on the website. Results will be e-mailed to you. This replaces the Virginia State Police criminal history record. All caregivers and household members over the age of 18 must complete fingerprinting no later than September 30, 2018. Background checks are now valid for five years from the original date the check was completed.
An unannounced renewal inspection was conducted with provider. There were seven children present, ranging in age from 4 months to 8 years, totalling 16 points in care. One infant was sleeping and the remaining children were dancing and playing with age appropriate toys/activities. Three children's files, three household files, and one assistant file were reviewed - please see Supplemental Summary for missing items. Provider does not administer medication - Medication Administration Training is required to administer medication. The provider renewed first aid and CPR certification 3/23/2018. Standard #: 22VAC40-111-100-A Description: Repeat violation: Based on review of three children's files, the provider failed to have documentation of a physical examination within the first 30 days of each child's attendance. Evidence: Child C did not have documentation of a physical examination on file within the first 30 days of attendance. Plan of Correction: Provider will request the information from the parent again.
Standard #: 22VAC40-111-50-A Description: Repeat violation: Based on observation, the provider failed to have a written record of children in attendance each day. Evidence: the attendance record was completed by the provider during today's inspection at approximately 12:00 p.m. Plan of Correction: Provider completed the attendance record during today's inspection.
Standard #: 22VAC40-111-70-A Description: Repeat violation: Based on review of three children's records, the provider failed to provide all required information in writing to the parents before the child's first day of attendance. Evidence: Child A did not have documentation of a completed VDSS form "Policy on the Administration of Medication", "Information for Parents", or "Provisions of the Emergency Preparedness plan" or an equivalent. Plan of Correction: Provider will provide the information to the parent and will get the parent signature.
Standard #: 22VAC40-111-170-A Description: Based on review of one assistant file, the provider failed to obtain a tuberculosis test or screening from each caregiver at the time of hire. Evidence: Assistant A (date of hire 2/2018) did not have documentation of a tuberculosis test or screening on file. Plan of Correction: Assistant A will get documentation of a tuberculosis test or screening.
Standard #: 22VAC40-111-180-A Description: Repeat violation: Based on review of three household files, the provider failed to have documentation of repeat tuberculosis test. Evidence: Household member 1 does not have a repeat tuberculosis test (expired 10/2/2017). Plan of Correction: Household member 1 will get a repeat tuberculosis test this week.
Standard #: 22VAC40-191-40-D-5-a Description: Repeat violation: Based on review of household files, the provider failed to have a child protective services central registry check within 30 days of a person in the home becoming 14 years old. Evidence: Household member 3 turned 14 in May 2017 and does not have a completed child protective services central registry check on file. Plan of Correction: The provider mailed the search of the central registry check to Central Registry on 7/2/2018. It was returned to provider on 7/9/2018 to correct errors. The errors will be corrected and provider will send the form again.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of one assistant file, the provider failed to have documentation of a completed search of the central registry (CPS) for each employee within 30 days of employment. Evidence: Assistant A (date of hire 2/2018) did not have a completed search of the central registry. Plan of Correction: The search of the central registry for Assistant A was mailed 7/2/2018. It was returned to the provider for corrections. Assistant A will not return to work until the CPS check is returned.
