Discussed Provider's sleeping policy versus parental permission and organization of documentation.
An unannounced non-mandated inspection was conducted today from 9:45am to 12:30pm. 3 children (11pts) were present with 1 caregiver. Ratios were in compliance. The physical space, 2 children's records, Provider training certificates, attendance records and evacuation drills were reviewed. Required documents were posted. No medications were on site. Children were observed in free play, napping and engaged with the Provider. Interactions were positive. The space was well organized and contained an abundant supply of developmentally appropriate materials. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-700-A Description: Based on record review, documentation of authorization to administer prescription or non-prescription medication was not available in 1 out of 2 children's records. Evidence: Child #1 was missing the written authorization forms required to administer prescription or non-prescription medications. Plan of Correction: I will request the information needed.
Standard #: 22VAC40-111-50-A Description: Based on records review and observation, the Provider did not keep a written record of children in attendance each day. Evidence: The attendance record was blank for today and 3 children were present. Plan of Correction: I will make sure to write attendance daily.
Standard #: 22VAC40-111-60-B Description: Based on record review, 1 out of 2 records were incomplete. Evidence: Child #1 Missing written authorization for emergency medical care, addresses for 2 emergency contacts, physician's name/address/phone number, medical insurance provider and medical insurance policy number. Plan of Correction: I will request the required information.
Standard #: 22VAC40-111-70-A Description: Based on record review, 2 out of 2 children's records did not contain written provisions of the family day home's emergency preparedness and response plan. Evidence: The documents for Child #1. Child #2 were missing. Plan of Correction: I will obtain the required information.
Standard #: 22VAC40-111-80-B Description: Based on record review, the Provider did not obtain proof of identity for 2 out of 2 children. Evidence: Child #1 and Child #2 were missing documentation to verify proof of identity. Plan of Correction: I will obtain the information right away.
