Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 12 children, totaling 30 points. Children's records, background checks, caregiver training, emergency preparedness items, the physical plant, and children's nutrition were inspected today. The provider reported that no children's medications were on site today. This inspection took place in the middle of the day. The children were observed listening to stories and taking their afternoon nap. The children were served chicken nuggets, mixed vegetables, watermelon, french fries, and milk for lunch today. Standard #: 22VAC40-111-920
Description: Based on observations, it was determined that the provider has not posted the menu as required. Evidence: The provider serves meals and snacks. The provider has not posted a menu for the current one week period.

Plan of Correction: My printer broke this week so I couldn't print the menu. I will handwrite the menu until my printer is fixed.

Standard #: 22VAC40-111-100-A
Description: Based on review of children's health record, 1 out of 5 files were missing current physical within 30 days after the first day of attendance. Evidence: Child #3 was missing a current health record. Child's first day was 3/19/18

Plan of Correction: Provider emailed the parent to request current physical during this inspection.

Standard #: 22VAC40-111-60-B
Description: Based on observation, 1 out of 5 children's files were missing required documentation. Evidence: Child #3 was missing 1 Emergency Contact name, address and phone number; and notification from the parent for Allergies and Emergency Action Plan.

Plan of Correction: I will ask the Parent for the information today.

Standard #: 22VAC40-111-70-A
Description: Based on review of children's files, 2 out of 5 files were missing the Provisions of the Emergency Plan. Evidence: Child#1 and Child#5 were missing Provider's Plan on Emergency Relocation procedures.

Plan of Correction: Provider will provide parent's with this information today and get the signature.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver health records, it was determined that the provider has not received a repeat TB Health Screening within the past 2 years. Evidence: The provider's TB Health Screening expired on 4/1/18.

Plan of Correction: I will call my doctor for the documentation. If I can't get the documentation, I will get another TB test done.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one area of the outdoor play area has not been maintained as required. Evidence: The air conditioning unit is on the ground at the children's level in the backyard play area. The metal covering on top of the unit is rusted.

Plan of Correction: I will put a portable fence up around the unit to enclose the area.
