Technical Assistance:
The caregivers have stated that they completed the fingerprinting process in June of 2018 but have not yet received their results. They have been encouraged to contact the Office of Background Investigations to inquire about the results of their fingerprints. OBI can be contacted at: 804-726-7884

Today an unannounced Monitoring inspection took place between 9:45am and 11:15am. There were 9 children present with the provider and 1 assistant for a total of 17 points within the supervision guidelines. The children were observed playing freely inside with play doh, flipping through various books and using an electronic tablet. The children went outside to play on the bikes before starting to rain. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Description: Based on record review, the provider did not document a written record of attendance each day. Evidence: At the start of the inspection, the last documented record of attendance was from 8/9/2018.

Plan of Correction: We'll do it everyday as the children arrive.

Standard #: 22VAC40-111-60-A
Description: Based on record review, children's records did not contain all required information. Evidence: Child #5 file did not contain documentation of an annual review of the child's emergency contact information.

Plan of Correction: We'll have the parents do it today.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain updated immunizations every six months for children under the age of two. Evidence: Child #4 (23 months old) most recent documentation of immunizations are from when the child was 1 year old. Child #5 (21 months old) most recent documentation of immunizations are from when the child was 5 months old.

Plan of Correction: I'll ask the parents tonight to bring me an update from her most recent check-up.
