Discussed diaper cream forms.

Comments:
An unannounced monitoring visit was conducted today between the hours of 9:00am through 11:00am with the provider on site. There were 5 children (11 points) with the provider and assistant. A sample of 5 children's files and 7 staff/household member's files were reviewed. The children were engaged in the following: toy play and outside play. The home is clean and organized. Violations:
Standard #: 22VAC40-111-890
Description: Based on observation, meals and snacks brought from home did not meet all requirements. Evidence: 1. Child A and Child B had meals from home in the refrigerator, but the food container was not labeled in a way that identifies the owner.

Plan of Correction: I labeled both meals and will make sure they get labeled each day.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child E (start date 4/12/2018) did not have a physical examination in the provider's file and has attended more than 30 days.

Plan of Correction: I will ask the parent today and give them the paper.

Standard #: 22VAC40-111-180-A
Description: Based on review, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1. Assistant B (Date of hire 11/2008) did not have a current tuberculosis screening form in the file.

Plan of Correction: I will get a copy of it.
