An unannounced renewal inspection was completed on 8/20/2018. The inspector arrived at 10 A.M. and departed at 12:30 P.M. Three children were in care at seven points with the licensee and the assistant . Two caregiver records and one household member's record were reviewed. Three children's records were reviewed. A physical plant walk-through was done. Policies and procedures were reviewed. Discussion: two year license, documentation of time off for the assistant, and house hold children.
Standard #: 22VAC40-111-180-A Description: Based on review of three records, the provider failed to ensure that all TB screening were completed every two years. Evidence: 1. The provider's TB was completed 1/7/2016 and the most recent was competed 6/5/2018. The household member's TB screening was completed 11/4/2015 and the most recent was completed on 6/10/2018. The assistant's TB screening was completed on 2/12/2018 and the most recent TB was completed on 6/15/2018. Plan of Correction: In the future all TB screenings will be conducted within the 2 year time frame and will be monitored.
Standard #: 22VAC40-111-530-B Description: Based on observation, the provider failed to ensure that the crib in use had a tight-fitting bottom cover. Evidence: 1. The crib in use had a loose fitted crib sheet. Observed an infant sleeping in the crib. Plan of Correction: The provider will purchase a smaller crib sheet instead of a regular size one.
Standard #: 22VAC40-111-590-A Description: Based on observation, the provider failed to ensure that infants were placed on their backs when sleeping. Evidence: 1. An infant under the age of 12 months was placed on their belly for nap time. The infant was patted on their back until asleep by the provider. The child was in sight and sound supervision with the provider during the nap time. Plan of Correction: The provider will place the infant on their back from now on.
