Discussed with provider about printing out fingerprinting results and keep on file.
An unannounced monitoring inspection was conducted today between the hours of 8:30 am through 10:00. There were a total of 2 children (6 points) with 1 caregiver. A total of 3 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, educational learning movie and snack. Standard #: 22VAC40-111-60-B Description: Based on record review, the provider has not maintained a complete record for each child. Evidence: Child #1 record does not have documentation of the medical insurance policy number and a second emergency name, address and phone number. Plan of Correction: I will ask the parent for this information.
Standard #: 22VAC40-111-690-D Description: Based on observation the changing table surface is not nonabsorbent. Evidence: The changing pad has 3 tears in the surface making the surface non absorbent. Plan of Correction: I will purchase a new changing table pad.
