Technical Assistance:
Consultation on the children's daily schedule. You will need to contact the Office of Background Investigation (OBI) at 804-726-7884 and they will provider you with Fieldprint codes and will email you your Facility ID that is assigned to you. You will need both the Code and ID number to schedule an appointment for the fingerprint background check. Below is information about obtaining that background check on the three adult household members. The fingerprint checks must be completed by 9/30/18. Below is information about how to schedule an appointment for fingerprinting. More details about this process will be added to: Where does an individual go to be fingerprinted? Fieldprint is the company DSS is working with to take fingerprints. You must make an appointment online. Fieldprint will not take walk in appointments. An unannounced monitoring inspection was conducted today with the provider, who was present with four preschoolers for a total of 8 points. The children were on their cots napping/resting at the beginning of the inspection. The provider reported that the children were very tired today so she fed them early ( spaghetti with a tomato meat sauce, bananas and milk). The provider does not administer medication and there was no medication on site for any of the day care children. Four children and three household member's files were reviewed today. Arrival time: 10:35 am Departure time: 11:50am

Description: Based on a review of record keeping and an interview with the provider, it was determined that shelter-in-place procedures are not practiced a minimum of twice per year. Evidence: The last documented shelter-in-place drill was dated 12/6/16.

Plan of Correction: I will remember to do shelter-in-place drills twice and year and keep documentation. I will do a shelter-in-place drill by the end of the week and another before the end of the 2018 year.

Standard #: 22VAC40-111-320-A
Description: Based on observation, it was determined that a small electrical appliance was left plugged into the outlet. Evidence: The Licensing Inspector observed a toaster plugged into the outlet. The toaster was not being used at the time.

Plan of Correction: I unplugged the toaster during the inspection.
