An unannounced monitoring inspection was completed on August 22, 2018 from approximately 11:30a.m. to 2:00p.m. Ten children were in attendance, infant to two years of age, for a total of thirty-one points. The children were observed interacting with the provider and two other caregivers, during diapering, lunch and nap times. Children?s toys, learning materials, equipment, bathroom, outside play area and the home?s first aid kit and emergency supplies were inspected. Five children?s records, four caregiver records, emergency preparedness plan, fire drill and shelter in place logs, daily attendance and required postings were reviewed. Transportation is not provided and medication is not administered. There were no injury reports to review. The provider was available for interview, inspection and exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were eight citations for violations of the Standards. See the violation notice on the Department's public web site for violations of the Standards.
Standard #: 22VAC40-111-800-A Description: Based on a review of records, the licensee failed to have a written emergency preparedness and response plan. Evidence: 1. Caregiver #1 was unable to produce an emergency preparedness and response plan. 2. Caregiver #1 indicated an emergency preparedness and response plan has not been created. Plan of Correction: Licensee will complete an emergency preparedness and response plan.
Standard #: 22VAC40-111-960-E Description: Based on observation, the licensee failed to ensure that bottles were labeled with the child?s name and date if more than one infant is in care. Evidence: 1. Three different types of bottles were observed, two on the kitchen counter and one being used by Caregiver #1 to feed an infant, with no names or dates labeled on them. 2. Caregiver #1 indicated that three children in care use bottles and acknowledged the bottles were not labeled with the date or children?s names. Plan of Correction: Licensee will label all bottles.
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, the licensee failed to ensure caregiver records contain all the required elements for two of four caregivers. Evidence: 1. Caregiver #3 was hired on March 26, 2018 and Caregiver # 4 was hired in December 2009. Caregiver #3?s record lacked an emergency contact, job title, age verification and the date of hire. Caregiver #4?s record lacked a job title, age verification and date of hire. 2. Caregiver #1 acknowledged this information was missing from these records. Plan of Correction: Licensee will obtain the missing information from the caregivers.
Standard #: 22VAC40-111-200-A Description: Based on a review of caregiver records, the licensee failed to provide orientation to the substitute provider and assistant by the end of their first week of assuming job responsibilities for one of four caregivers. Evidence: 1. Caregiver #3 was hired on March 26, 2018. 2. Caregiver #3?s record lacked documentation of any orientation prior to Caregiver 3?s first week of employment or before assuming job responsibilities. 3. Caregiver #1 acknowledged this orientation was not provided. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-111-210-A Description: Based on a review of caregiver records, the licensee failed to ensure caregivers obtained 16 clock hours of annual training for two of four caregivers. Evidence: 1. Caregiver #2 was hired in January 2010 and Caregiver #4 in December 2009. 2. Caregiver #2?s record reflected two hours of annual training for 2017 and Caregiver # 4?s record lacked documentation of any annual training. 3. Caregiver #1 acknowledged annual training was not completed as required. Plan of Correction: Licensee will ensure that all caregivers get the proper amount of training.
Standard #: 22VAC40-111-570-A Description: Based on observation, the licensee failed to ensure that a caregiver does not exceed 16 points when determining if an additional caregiver is needed.
Evidence: 1. Upon entering the home, ten children were observed in care, with one caregiver for approximately thirty minutes before an additional caregiver arrived. 2. Caregiver #1 reported the children in care included five two years old, three one years old, two infants and an additional child, seven years old, residing in the home, totaling thirty-one points. 3. Caregiver #1 acknowledged an additional caregiver was needed.
Plan of Correction: Licensee will ensure points do not total 16 or more with one caregiver or there will be another caregiver.
Standard #: 22VAC40-111-590-A Description: Based on observation, the licensee failed to ensure that infants were placed on their backs while sleeping unless otherwise ordered by a physician.
Evidence: 1. Upon entering the home, Child #2, two months of age, and Child #1, nine months of age, were observed sleeping face down, positioned on their stomachs. 2. Child #1 and Child #2?s records lacked documentation of a written statement by a physician ordering this sleeping position. 3. Caregiver #1 acknowledged Child #1 and Child#2 were sleeping on their stomachs and that there was no documentation from a physician ordering this.
Plan of Correction: Licensee will obtain written documentation from the parent if its necessary for an infant to sleep on their stomach or will no longer do this.
Standard #: 63.2-1720.1-B-2 Description: Based on review of caregiver records, the licensee failed to obtain a criminal history record check prior to employment for one of four caregivers. Evidence: 1. Caregiver #3 was hired on March 26, 2018. 2. Caregiver #3?s record contained a criminal history record check dated May 4, 2018. 2. Caregiver #1 acknowledged Caregiver #3 began employment prior to obtaining the results of a criminal history record check. Plan of Correction: Licensee will obtain criminal history record check findings before people are hired.
