Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:00pm. There were 6 children (19 points) directly supervised by 2 staff. At the start of inspection, the provider was not home. A substitute provider was caring for the children in the home. The home, 1 staff records, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, completing puzzles, doing a read aloud in Spanish, singing and dancing, and celebrating a potty break for a child potty training. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not ensure all children's files had the required information. Evidence: Child 1 is missing a second emergency contact.

Plan of Correction: The parents stated they did not have one. But I will make sure to get lunch.

Standard #: 22VAC40-111-570-A
Description: Based on observation, the provider exceeded 16 points per caregiver. Evidence: Upon arrival to the home, the provider was not in the home. The assistant was present in the home caring for 6 children (19 points). The provider stated she was away from the house for about 20 minutes running an errand.

Plan of Correction: The provider arrived and was in compliance. I will ensure an additional assistant is in the home or ask for help with errands. This is not a usual occurrence.
