Discussed with the provider: Limit television watching to 2 hours per day; Nothing is to be in the crib with the baby; one family member's fingerprint results are to be on file by Septemeber 30; Emergency escape drills are to be done every month, Shelter in place drills are to be done two times a year.
An unannounced monitoring inspection was conducted beginning at 9:45 and ending at 11:45. There were 3 children in care, ages 4 months, 2 years and 3 years. The children were observed watching children's television program and playing with toys. Reviewed family and children's files - see violation notice. Standard #: 22VAC40-111-60-B Description: Based on a review of the records for the three children in care, it was determined that the records are not complete. Evidence: The records for Child #1, #2 and #3 are missing Work Number for parent, Empergency preparedness plan notification, and medication policy. The record for Child #2 is missing medical insurance policy number. The record for Child #1 is missing page two of the Information for parents. Plan of Correction: The provider will complete all files.
Standard #: 22VAC40-111-280 Description: Based on observation, it was determined that there were hazardous substances in the bathroom used by the children. Evidence: There was a liquid soap and hand sanitizer that has a label that says Keep out of the reach of children. They are on the sink accessible to the three year old. Plan of Correction: They wull be kept locked except when in use.
Standard #: 22VAC40-111-510-A Description: Based on observation it was determined that the two older children do not have individual resting/napping place Evidence: The two children, ages Two years and three years are sleeping in one bed. Plan of Correction: The children will have individual mats or cots.
Standard #: 22VAC40-111-590-A Description: Based on observation and interview with the provider, it was determined that the infant is not placed in the crib on her back. Evidence: Observed 4 month old in the crib. she was on her back. Thwe provider stated that she does not roll over by herself. Plan of Correction: All children who cannot roll over on their own will be placed in the crib on their back..
Standard #: 22VAC40-111-690-D Description: Based on observation it was determined that the surface used for changing the baby is also used by other children . Evidence: The infant is changed on the bathroom sink where the other children wash their hands. Plan of Correction: The baby will be changed on a mat on the floor in the bathroom.
