Technical Assistance:
Provided the phone number for central registry to follow up on central registry finding for assistant. Reminded Provider that she can not give medications until she has renewed her MAT, many medication forms on file. Use Child Record form (2 page form) for child information, no need to have the form "Information and Agreement", (3 page form, dated 6/99) completed also. Provided copy of list of trainings for Provider.

Comments:
An unannounced renewal inspection was conducted today between the hours of 9:00 am through 11:00. There were a total of 9 children (23 points) with 2 caregivers. A total of 6 children's files and 6 staff/household member's files were reviewed. The children were engaged in the following: toy play, blocks, snack, snack, music and dance. Provider stated there were no children medications. Discussed the importance of obtaining the central registry finding for the assistant before she returns to work. Standard #: 22VAC40-111-60-B
Description: Based on record review, each child's record is not complete. Evidence: Child #1, date of first attendance 8/21/18 is missing medical insurance name and policy number and the child's immunization record. Child #2, date of first attendance 8/8/18 is missing authorization for emergency care.

Plan of Correction: I will get the missing information from the parents.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the provider has not obtained an original central registry finding within 30 days of employment for each caregiver. Evidence: Assistant #1, date of hire 9/23/2016 does not have a current central registry finding within 30 days of employment.

Plan of Correction: The form was corrected and resent. Provider left a message for central registry to inquire about the form. The assistant has not worked here since June 2018. She will not return until the form is received.
