Technical Assistance:
Provided the field print number to obtain a copy of the fingerprints. (877-614-4364) and OBI ( 804-726-7884. Provider stated that she completed her fingerprints back in February 2018, with no results received.

An unannounced monitoring inspection was conducted today between the hours of 10:00 am through 11:30. There were a total of 5 children (18 points) with 2 caregivers. A total of 4 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play and play dough. Discussed with the Provider concerns with waiting for more than 6 minutes to open the door. Both Provider and assistant were in the home with 5 children. Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider has not obtained documentation of a physical examination for each child within 30 days after the first day of attendance. Evidence: Child #1, first day of attendance 5/7/18 and Child #2, first day of attendance 7/9/18 does not have documentation of a physical examination on file.

Plan of Correction: I will contact the parents for this information. The forms will be emailed.

Description: Based on record review, each child's record is not complete. Evidence: Child # 2, first day of attendance 7/9/18 does not have documentation of the medical insurance policy number.

Plan of Correction: I will contact the parent for this information.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were found accessible to children. Evidence: Two potentially poisonous substances were found on top of the play kitchen sink in the sleeping room. One child climbed onto the chair where she had access to the substances.

Plan of Correction: The assistant immediately removed the substances and locked it up.

Description: Based on observation, outlet covers are not installed on all unused electrical outlets. Evidence: One electrical outlet under the windows was found uncovered in the sleeping room.

Plan of Correction: The outlet was covered.

Standard #: 22VAC40-111-540-A
Description: Based on observation, children are not being securely fastened using a safety strap when the infant feeding tables are being used. Evidence: Upon the licensing inspector's arrival, child # 1, 10 months old was observed in a feeding table. The safety strap was not securely fastened.

Plan of Correction: We will keep all children fastened while in the seats.
