Comments:

Licensing Inspector conducted an unannounced Religiously Exempt Code Annual Inspection on October 24, 2023. There were 91 children ages 14 months to 5 years old present under the direct care of 12 staff. The Licensing Inspector reviewed 5 children?s records and 6 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	22.1-289.035-A
Description:	Based on record review, the center failed to ensure required Background Checks are completed every 5 years for every staff person. EVIDENCE: Staff #1 did not have an updated Central Registry Background Check within five years from the most recent Background Check completed on 9/01/2015. Staff #3 did not have an updated Central Registry Background Check within five years from the most recent Background Check completed on 1/26/2018. Staff #5 did not have an updated Central Registry Background Check within five years from the most recent Background Check completed on 4/26/2017.
Plan of Correction:	Center Director will submit documents to update Background. Documentation of the findings will be maintained in the staff records.
Standard #:	22.1-289.035-B-4
Description:	Based on record review, the Center failed to obtain required out-of-state Background Checks for staff who are lived out of state within the past five years from the date of hire. EVIDENCE: Staff #2 lived outside of the state (New York) within 5 years before date of hire (2/27/2019), record did not include out-of-state Background Checks. Staff #5 lived outside of the state (North Carolina) within 5 years before date of hire (1/30/2022), record did not include out-of-state Background Checks.
Plan of Correction:	Out-of-state background checks will be submitted for each staff person who has lived outside of the state. documentation of the findings will be maintained in the staff record.
