Discussed crib mattresses- only one mattress may be kept in a crib. Be sure parent documents that child does not have a milk allergy.
An unannounced inspection was conducted from 830-1030 AM. There were 8 children ( 11 months- 4 years) with 2 caregivers for a total of 23 points. The Provider and assistant are CPR/First Aid certified and are currently enrolled in MAT certification course. Provider and assistant have been attending workshops and have certificates of over 10 hours. Program, diapering, hand washing, and feeding were observed. Please call with questions 703-479-4679
Standard #: 22VAC40-111-60-B Description: Based on record review, the Provider did not maintain complete file for 1 of 3 children's files. Evidence: 1. Child #2's file has an Emergency preparedness acknowledgement form but the relocation address is not recorded. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-111-180-A Description: Based on record review and interviews, the Provider did not obtain TB Screenings at least every two years from the date of the last screening for herself and a household member. Evidence: 1. TB Screening documents reviewed for Provider and household member had a date of July 7, 2016. 2. Provider stated she thought she had done them but call to Dr verified last screening was July, 2016. Plan of Correction: I am scheduled for Monday and will have husband do one when he comes back.
Standard #: 22VAC40-191-40-D-5-a Description: Based on record review, the Provider did not obtain a completed CPS check for a household member who turned 14 years old. Evidence: 1. The CPS form for household member #2 has documentation to verify it was received by the Central Registry office but has a blue mark to indicate it was not complete. Plan of Correction: I will have him complete a new one today and send out today.
