The provider was reminded that she must update the menu posted to reflect the meals that were served before the end of the day and encouraged to drop a crib mattress for child #1 to a level that is more developmentally appropriate.

Today an unannounced Monitoring inspection took place between 10:15am and 1:25pm. There were 10 children present with the provider and 1 assistant for a total of 28 points within the supervision guidelines. The children were observed playing freely, singing songs, watching Pete the Cat videos on the provider's phone, washing hands and eating lunch (peanut butter sandwiches and milk). The provider's assistant is absent today and her daughter is filling in to assist. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-880 Description: Based on observation, the provider did not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the USDA. Evidence: The children were only served a peanut butter and jelly sandwich for lunch. There were not vegetables offer to the children. The jelly on the sandwiches did not meet the minimum requirements for the amount of fruit to be served and the amount of peanut butter on the sandwiches did not meet the minimum requirements for the meat alternate. After the children left the table they were offered bananas, however only three out of six children returned to the table for bananas slices. Plan of Correction: I will do that. I always do it but today was a busy day.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information. Evidence: Child #3 file did not contain the families medical insurance policy number or the street address for their second emergency contact. Plan of Correction: I'll tell them today.
Standard #: 22VAC40-111-530-A Description: Based on observation, rest mats used at nap time did not contain a bottom cover. Evidence: Seven out of seven children were observed napping on rest mats did not contain a bottom linen. Plan of Correction: I didn't know. I'll buy them.
