Technical Assistance:

During the inspection, the licensing representative discussed with the Provider diaper creams and documenting injuries. Delayed entry into the home for the licensing representative to conduct an inspection was also discussed.

Comments:

An unannounced renewal study was conducted today from 10:00am-12:00pm. A total of 10 children [29 points] with 3 caregivers. Infant feeding, potty training and mealtime were observed during the inspection. A sample of 10 children's records, 5 caregiver/household member records, emergency drills, emergency supplies, and other documents were reviewed. Description:	Based on observation, medications were not kept in a locked place. Evidence: An over-the-counter medication was observed in an unlocked medicine cabinet in bathroom B.
Plan of Correction:	Medications were removed from the medicine cabinet during the inspection.
Standard #:	22VAC40-111-960-E
Description:	Based on observations, an infant bottle was not labeled with a date when more than one infant was in care. Evidence: During the inspection, 4 infants were observed in care. 2 children's bottles were not labeled with a date.

Description:	Based on records reviewed, 3 out of 10 children's records were not maintained up-to-date. Evidence: 1. The most recently documented record update for Child #4 was obtained 10/2/2017. 2. The most recently documented record update for Child #6 was obtained 8/12/2017. 3. The most recently documented record update for Child #7 was obtained 10/10/2017.

Description:	Based on records reviewed, 2 out of 10 children's records did not include the required information. Evidence: The record for Child #1 and #3 was missing 1 of 2 required emergency contacts.

Description:	Based on observation, hot water temperatures available to children exceeded maximum range of 120?F. Evidence: 1. The water temperature in bathroom A used for children in care was 121.5?F. 2. The water temperature in bathroom B used for children in care was 122?F.
Plan of Correction:	We contacted a private company that will adjust the water temperature.

Description:	Based on records reviewed, applicant fingerprint results were not completed as required by September 30, 2018. Evidence: The fingerprint results for the applicant were received 10/5/2018.
