Comments:
An unannounced renewal inspection has been conducted from 930-11:00 AM. There were 8 children present with 2 caregivers for a total of 21 points. The Provider is preparing to start a preschool program in January but already provides curriculum for the children. Files were reviewed, emergency drills were reviewed, training documentation was viewed and all caregivers are First Aid/CPR certified. The Provider is not MAT certified and states no one is on medication or have possible emergency medication needs. Please call with questions 703-479-4679

Standard #: 22VAC40-111-180-A
Description: Based on record review, the Provider did not obtain current TB Screening reports every two years from the date of the last screening. Evidence: 1. Caregiver records reviewed, and Provider, assistant #1, and #2's TB Screenings were all completed in August, 2016.

Plan of Correction: I have made an appointment, my husband has been to have the screening, and I will make sure assistant goes.

Standard #: 22VAC40-111-240-A
Description: Based on observation and interview, the Provider has a piece of equipment that is not safe. Evidence: 1. Provider stated she has a small trampoline and it is used by the children. The Trampoline is in the back play area with the other equipment and has easy access for child's use.

Plan of Correction: I will remove it from that area and only allow grandchildren to use but not during operating hours.
