Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the changes needed for the upcoming license in order to reflect its new address. The new address is: 745 Bonnie Ridge Drive, N.E., Leesburg, Virginia 20176-3619. The home is in the process of obtaining official documentation from the Town of Leesburg to establish their posted capacity. Both the age range and hours of operation shall remain the same. Discussion of the FDH Emergency Preparedness & Response Plan based on the new home address. Discussion of Child #4 asleep today in a bouncer. When possible, please transfer 15 month old child to a cot or mat. Provider stated that the bouncer held calm a child down when they are fussy.

The following inspection took place between the hours of 12 pm (noon) through 3 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in the town of Leesburg, Virginia. At the start of this unannounced renewal inspection there were a total of 6 children (19 points) with 2 caregivers. A total of 10 children?s files, and 2 staff files were reviewed. The children were engaged in the following: freeplay, toy play, music, nap time, bathroom break, and other organized activities. Description: Based on review, 1 of 10 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (11/02/2018), an updated immunization page for Child #3 was not available for review. 2. The most current immunization documentation on file for Child #3 was dated - 02/15/2018. 3. Based on the information on file, on the date of inspection, Child #3 was under two years of age.

Plan of Correction: Updated documentation shall be obtained for Child #3.
