Comments:
Thank you for your time and assistance with the announced initial inspection conducted today, November 7, 2018. The on-site inspection was held from 2:00p.m. to 5:30p.m. On this date, there were four children in the care of the provider and a total of five children enrolled in the program. There were four children's records reviewed, along with the records for the provider and two household members. There was no medication on-site or being administered at the time of the inspection. The children brought their lunch from home and the morning and afternoon snacks were provided by the provider. All interactions between the provider and the children were age-appropriate and positive. The home and outside play areas were toured. There was an ample supply of toys and equipment for the children. The exceptions to compliance were recorded on the violation notice and the risk ratings for the violations were noted on the supplemental page. Some items reviewed and observed included: required posted information, emergency preparedness plan, refrigerator and freezer temperature, emergency supplies, first-aid kit, warm water temperature, daily attendance record, nap time, play time, snack time, diapering changing procedures, bathroom and hand washing procedures and dismissal procedures. A recommendation will be made to issue a conditional family day home license with a capacity of four children and an age range of 16 months through 12 years. Violations:
Standard #: 22VAC40-111-770
Description: Based on observation and verification from the provider, the provider failed to have a battery-operated portable weather band radio. Evidence: 1. The weather band radio the provider had available was not battery-operated.

Plan of Correction: I will order a battery-operated radio tonight.

Standard #: 22VAC40-111-320-B
Description: Based on observation and confirmation from the provider, there were unused electrical outlets which did not contain protective covers. Evidence: 1. There were 11 unused electrical outlets in the kitchen which were not cover with protective covers. 2. There were three unused electrical outlets in the bathroom which did not contain protective covers.

Plan of Correction: I will order more protective outlet covers tonight.
