Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on December 14, 2023. There were 5 children ages 2 to 3 years old present under the direct care of 2 staff. The Licensing Inspector reviewed 2 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.058 Description: Based on inspection, the Center failed to have a carbon monoxide detectors for each building that was built before 2015 and houses a child day program.
EVIDENCE:
A carbon monoxide detector was unavailable.Plan of Correction: Director will purchase and install a carbon monoxide detector.
Standard #: 8VAC20-770-40-D-2 Description: Based on record review, the Center failed to ensure each staff person who is expected to be alone with one or more children enrolled in the religious exempt child day center complete a sworn statement or affirmation
